There are other people working for Kaiser than MDs, and other means of
promotion than face-to-face exhortation from doctors.  In the face-to-face
exhortation department, there are health educators who aren't MDs and make
less money.  Kaiser also publishes all kinds of pamphlets (and often makes
them available free via web, even to nonsubscribers).
