I've heard of intentional spread of disease (which should count), but 
haven't found good references for that.

Japan & Germany were the aggressors. As well, the response was by an 
alliance of nations, not just the US (and Canada & Britain participated 
to an extent in the production of the nuclear weapons). While the Allies 
aren't blameless for civilian casualties, Japan & Germany are arguably 
responsible as well.

Speaking of nukes, I don't buy the high death toll estimates from 
nuclear weapons production and testing, but I expect there were some.
