A short-lived show called "Tiger Team" that did penetration testing 
of security systems touched on that on the jewelry store job. Took 
one look in the ceiling, and came to the conclusion that no human 
being could possible pass through. They didn't seem the least bit 
suprised.

Movies have human sized air ducts because writers are idiots, or 
directors are idiots, or producers are idiots, or all of them are 
idiots, and they just can't think of a better way to advance the 
plot.
