At the predefined targets.  There was talk of de-targetting the
nuclear weapons a few years ago, so maybe there isn't pre-defined
targets anymore.

A launch at China and Russia would seem to be the most likely outcome
if they decide to fire.  Ofc, that would likely trigger a counter
strike (and in theory, they might actually be the ones responsible).

Is this supposed to be an attack by a 'mad scientist' or another
country ?  The whole MAD policy would seem to act as a disincentive
for this attack type too.

If they did figure that the attack used radio waves, then EMP-ing the
entire home country might be worth a shot, that would knock out nearly
all radio signals for hours and should get a reasonable number of the
locals awake, while not actually damaging the country.  In fact, it
might take out the satellites if this is what is being used to
transmit the signal.
