Wikipedia.  And it has all the problems you list later in your post,
and more.  They aren't unique to Wikipedia either - Everything2
suffered from many of them as well.

When I helped stand up the IOBA (a real world organization operated
over the 'net) precisely the effect you described happened as well.
The decision makers met face-to-face and the rest of us were presented
with a fait accompli - with all the downsides you describe.  What
could have been a very effective organization was hamstrung aborning.
