If you're going to use black holes, don't use a single black hole.
You can minimize the tidal effects by creating a large 2d plane.
This would involve a bunch of small black holes--one in the center,
and the rest in circular orbits around the center.  The bulk of
the star****p is in "front" of this plane, and accelerates the
black hole array forward by gravity.

The main star****p could be accelerated with a stupendously
powerful X-ray laser (which could have intergalactic range--a
0.83nm X-ray laser focused by a 40000km diameter aluminum
zone plate could focus onto a 1000km target out to four million
light years).

Rather than mess around with black holes, it may be easier
to use a combination of fluid immersion and superconducting
pinning, perhaps implanting tiny ferromagnets throughout
the victim's body.  This still leaves the more formidable
challenge of accelerating the star****p by a hundred gees for
a while.
