I don't read spoilers, and I figured that one out the moment Kate
mentioned she had a son.  

Really, if an episode is depending on a Shocking Twist!, it needs
to find a twist that will actually be at least mildly surprising to
people who understand things like economy of characters.

At very least, it needs to avoid scenes where it practically waves
a banner proclaiming, "Look here!  A character!  That neatly fills
the conspicuous opening from two scenes ago!  And which serves no
other obvious purpose!  But which we can't kill off!"  Not that we
needed the hint by then, but still...
